This webpage is for the Urban Indian Health Program Patient Centered Health Home (UIHP PCMH) collaborative, facilitated by Merin McCabe, CPC CPC-I ICD10 Instructor.
Members of this collaborative assist and support one another in either achieving or maintaining recognition as a Patient Centered Health Home, which can be earned from a variety of entities including NCQA and AAAHC. Payers and States also have recognition programs. The requirements are similar enough, so that we will be able to make progress regardless of the entity you choose.
This work group is established to assist one another to develop the policies, procedures and audits we all need. Our programs are similar; but, also very unique. Therefore our processes and procedures will be too.
Participation is voluntary. We will plan to meet via teleconference every on Fridays at 10:00 am Pacific Time. The link and phone number will be sent to members of the collaborative before this time. Click here to view the UIHP 2016 Schedule.
